Mayfield Medical Centres and Old Farm Surgery are proposing to merge from 1 January 2019, or as soon as practicable thereafter. The planned merger will allow the combined practice to thrive and provide the facilities and services that our patients deserve. Every member of staff is committed to making healthcare for our patients the best it can be, and it is our combined view that the merger will be positive for patient care.
We understand there may be concerns or ideas on how it can best work and we would like to encourage our patients to let us know via this survey.
Please read this for a bit more detail.
We want to keep you fully informed, hear your views and answer any questions you may have. The views of our combined patients are very important to us and will be taken into account during discussions over the coming weeks. Please print off this questionairre, fill it in & drop it in to the surgery. Or pick up a hard copy from the surgery. Thank you.

We are pleased to let you know that we are working closely with the Old Farm Surgery just down the road. For example, we have the same Practice Manager, Colin Whitford, and we are sharing a Paramedic (Ryan) who is helping with visits, as well as sharing some GP and Nurse resources and administrative support.
The NHS is constantly changing, and part of this are significant financial cuts, so we are doing this in order to ensure the long term sustainability of General Practice in this area.
